Passports

• Visit the U.S. Passport Form Wizard on the U.S. Department of State Website.

• Applicants must use the DS-11 form unless they qualify to use the DS-82 (confirm if the applicant meets the DS-82 requirements at Renew my Passport by Mail)
• DS-11 passport applications are accepted by appointment only, Monday – Friday 8:20 a.m. – 3 p.m.
• Applicants should read each question carefully to ensure it is answered correctly.
• The application must be
  • Completed in its entirety prior to the start of the appointment time
    • No field should be left blank, write N/A if the information does not apply or “unknown” if the applicant does not know the information.
    • If you are using the online form filler on the Department of State’s website, fill in all fields, even though they are not “required”.
  • Written in black ink
  • Printed single-sided
  • Free from mistakes and cross-outs
  • No whiteout

• Only applicants who meet the requirements may use the DS-82. Confirm if the applicant meets the requirements at the U.S Department of State website.
  • Those that do not meet the requirements must use the DS-11.
• DS-82 passport applications are completed and mailed by the applicant, without assistance from the passport facility.  DS-82 applicants may come to the city clerk counter to get a passport photo and obtain a DS-82 form.  Do not make an appointment if you are eligible to use the DS-82.

• Each applicant will be paying fees to two different entities.
  1. U.S. Department of State’s fees (application fee) – check or money orders are the only form of payment accepted; a separate check or money order for each applicant is required (if there are four applicants, four separate checks or money orders are required).
  2. City of Bloomington fees (photo and execution fees) – cash, check, or credit cards accepted, a separate payment is not needed for each applicant.

• The City of Bloomington provides passport photo services to both DS-11 and DS-82 applicants for a fee.
• Photos are taken by a trained passport agent using specialty passport software to ensure compliance with the stringent passport photo standards.

• Appointments are required for all DS-11 applicants. Walk-ins are not accepted.
• All applicants must be present and on time, have their required documents present, the correct forms of payment and their application completed at the start time of their appointment. Failure to do so will result in your appointment being rescheduled to a different day.
• Applicants that are more than five minutes late will be rescheduled to a different day.
• Applications will be processed for the number of people the appointment was made for (if the appointment was made for two people, we will process two people, not three).
• Passport appointments are in high demand. If there are not any available appointments, check back on Friday mornings when appointments for the new week are opened.
• Appointments are not required for DS-82 applicants who want their passport photo taken or are picking up a DS-82 form.  Please do not make an appointment if you are using the DS-82.

• The documents required for each applicant vary depending on that applicant’s particular circumstances.  It is the responsibility of the applicant to determine which documents they will need, given their circumstances.  Please carefully read the Prepare to Apply for a Passport Homepage to determine which documents the applicant needs.

• Minors must be present for the passport appointment.
• Both parents or guardians must be present at the appointment and approve passport issuance to a child aged 15 and under.  If one or both cannot apply with the child, more documentation is required.  See Apply for a Child's U.S. Passport for additional documents required.
• Minor applicants have different requirements.  Read all instructions carefully for minor applicants at Apply for a Child's U.S. Passport
• Applicants aged 16 and 17 must appear in person with one parent.

If a parent is being detained by ICE, please use form ds5525.pdf and provide as much documentation as possible.

• Check passport processing times.
• The City of Bloomington offers routine and expedited service.
  • If you have international travel within 14 days, you must make an appointment with the Minneapolis Passport Agency.

• Passport books and passport cards are Real ID compliant and may be used for domestic travel.
