COVID-19 vaccine info for healthcare professionals

The Metro Health & Medical Preparedness Coalition is providing COVID-19 vaccination support to metro-area health care professionals who are independent and are not affiliated with a hospital or health care system. The Bloomington Public Health Division is a member of the Coalition. If you are not connected to a health care system, please fill out this brief survey. The information will be sent to the appropriate agency to coordinate vaccination.

The Metro Health & Medical Preparedness Coalition is providing COVID-19 vaccination support to metro-area health care professionals who are independent and are not affiliated with a hospital. The metro region includes the following counties: Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington. The Bloomington Public Health Division is a member of the Coalition.

The Coalition has developed a survey that will help in compiling a list of health professionals who are not affiliated with a hospital.

Who will administer my vaccine?

  • If a hospital health care system employs you, they will administer your vaccination.
  • If you are affiliated with a health care system and provide direct patient care at one of their facilities, that health care system may vaccinate you. Contact the system(s) to verify you are included in their vaccine plan. The best contact may be the individual you contract with within the system.
  • If you are not connected to a health care system, please fill out this brief survey. The information will be sent to the most appropriate agency to coordinate vaccination planning.

Interested in becoming a vaccine provider?

You may be able to become a provider of vaccine by registering with the Minnesota Department of Health.
